Announced to the Action Program Participants in Period 2 who have been declared to have passed, that the LSP exam will be held to get a BNSP certificate, on:

For the Participants in the LSP Action Program for Period 2 is expected to complete the following document requirements:

  • Fill out the registration form on the link ( lsp.upy.ac.id ) in Save and printed.

    • For Username and Password Please click the following link ( Download Username and Password < /Strong> )

    1. Form Mak, please click the following link ( Download Form ), if it has been downloaded, please print/print.
    2. Color Photo Pas (3×4) 4 sheets (red background)
    3. Photocopy of KTP / Family Card (2 Sheets)
    4. Photocopy of Action Program Certificate
    5. Photocopy of the last diploma (1 sheet)
    6. Green Map (Written: Name, NPM and Group)
    7. Portfolio

    8. If you are working: Curriculum Vitae
    9. Student / Student: Photocopy of Student Card / Student Card

      10. Documents & Ndash; The document must be collected on 26-28 September 2018 during working hours at UPY PPTIK.

      If the participant does not collect one or does not collect at all is declared incompetent and declared dead.
